The consumer consents to the sharing of medicine-related information between the pharmacist and the consumer’s doctors, hospitals, or community nurse as relevant.
The consumer consents to a medication review and assessment of their management of the DAA every 6/12 months at the pharmacist’s discretion.The consumer or their carer is responsible for providing a valid prescription for each medication to be packed in the DAA.
If the GP is willing to provide a prescription for ongoing medication without a formal consultation we can contact the prescriber to organise any required prescription. If not, the pharmacy will remind the consumer or carer when a new prescription is required and will print out a new prescription reminder form if required.The GP is responsible for authorising changes to medicines. Any changes to medicines must be made in writing in the form of an updated medication profile. This is to optimise patient safety.
If changes to medicines in the pack need to be made before the current DAA pack is finished, please return the DAA pack to the pharmacy so that untaken medications can be re-packed into your new DAA pack as necessary. This will minimise the need to dispense extra prescriptions. If the DAA is lost, damaged, destroyed or opened incorrectly, contact the pharmacy as soon as possible, so replacement can be made. Any extra charge for replacement DAA packs is at the pharmacist’s discretion.
